Purpose of the role:
This pivotal role offers the opportunity to shape and drive innovative health and wellbeing strategies across our homes. You’ll be the driving force behind delivering excellence in dementia care, nutrition, physical health, and emotional wellbeing, leading trust-wide initiatives that enhance our residents’ health, happiness, and quality of life.
As a Head of Health, Wellbeing and Dementia, you will:
- Shape and deliver health and wellbeing priorities across the Trust, in collaboration with senior leadership, managers, and our in-home support services such as Activities, Catering, and Care Teams across the whole portfolio of the Greensleeves Care homes
- Design and support tailored exercise programmes for older adults, including falls prevention, strength, and balance activities.
- Conduct gap analysis of dementia provision and recommend improvements.
- Develop and lead health and wellbeing frameworks across our care homes.
- Oversee and innovate dementia care provision, embedding best practices and supporting environments
- Drive improvements in physical health through tailored activity programmes.
- Collaborate with chefs, dietitians, and care teams to meet individual dietary needs, including dysphagia, weight loss, or cultural preferences.
- Champion dignified and nutritional dining experiences
- Support and train teams to deliver exceptional resident-centred care.
- Build partnerships with health professionals and sector experts.
- Ensure compliance, audit quality and monitor key wellbeing KPIs.
To apply, you will need:
- Level 3 Personal Trainer qualification or equivalent (e.g., physical activity for older adults).
- Certification in nutrition particularly for older adults.
- Certificates in Health promotion, Gerontology, Dementia care, Exercise for older adult.
- Qualifications in Dementia Care Mapping
- Strong understanding of ageing-related health conditions, especially dementia, frailty, malnutrition, and sarcopenia.
- Knowledge of safe and appropriate physical activity interventions for older adults, including chair-based exercise, resistance training, and falls prevention.
- In-depth awareness of the Eatwell Guide, hydration, and how nutritional interventions support ageing well and managing long-term conditions
- Experience in elderly residential care, including dementia support.
- Strong knowledge of ageing-related health needs, including nutrition, frailty, falls prevention, and sarcopenia.
- Skilled in developing staff and leading multidisciplinary wellbeing programmes.
- Excellent leadership, communication, and organisational skills.
- Experience with quality assurance and regulatory compliance in care settings.
- Knowledge of the Eatwell Guide, mealtime social strategies, and sustainable programme delivery.
